For those who choose the 24-hour delivery, they are required to send a self-addressed stamped envelope together with their request. The Office sets no restrictions as to the number of records one can request.

The divorce form is available in three different languages, which one can easily download from the website. One can choose the default divorce and Annulment form, which is in English, or download the Spanish or Creole version of the form. All blank fields must be answered to prevent delay in the processing of the request.

If you are not sure about the complete name of the wife, you have to make sure that you include the complete name of the husband, as the divorce records are usually indexed under the husband's name only. The details of the person who requested the information should also be included as well as any necessary information like phone number and the mailing address the Bureau might need.
